Place name as presented at the auction. If you'd like to remain anonymous, write 'Anonymous'.

Write the title and description of your auction item donation(s).

What is the fair market value of your donated auction item(s)?

When talking photos of your auction item(s) please be sure to use bright lighting and center the items so they are clearly seen. Photos will be used in the online auction for bidders to get an idea of the item. Great photos encourage bidders! If it is a virtual item or service feel free to use a high resolution image of the related item logo. If you’d like your company’s logo to be thanked as a donor, please attach that as well.
